The basic unit of the Butler County Board of DD program design is the Community Team. Community Teams are intended to be both the initial and primary point of contact for individuals and families who are seeking services and supports through the DD program. The teams are located throughout the county in the Middletown, West Chester, and Hamilton/Fairfield communities. The community teams are committed to supporting and empowering individuals who use DD services so they can make decisions about how they can live their lives.
Most of the Community Team members are “Support Coordinators”. Each of these teams are comprised of a Team Supervisor, Team Secretary and a Support Coordinator that is primarily responsible for eligibility determination. In 2003, DD began assigning Early Intervention specialists to work as members of the different community teams. Team offices are located at the West Hamilton office on Eaton Ave. (across from Hamilton HS) in Hamilton, the Fairfield office located on Donald Dr. in Fairfield, the West Chester Team Office inside the Janet Clemmons Center on Fairgrove Avenue in Hamilton and the Middletown office on Lewis Street in Middletown.
Team members are responsible to:
- Establishing individuals’ eligibility for DD services.
- Developing plans for individuals so that they get the services they want and need.
- Helping people to become more connected in their communities.
- Establishing budgets for needed services.
- Helping identify qualified service providers so people can choose providers they like.
- Ensuring that services meet the needs of the people being supported, and revise services if they do not.
- Working with service providers to make sure that they are providing the supports that they have agreed to provide.
- Monitoring and ensure the health and safety of the people they help.
- Helping people manage their money so they can figure out what they can afford.
- Ensuring that each individual served has a designated person to provide representation, advocacy, advice and assistance related to the day to day coordination of services in accordance to his/her plan.
- Assisting families to make future life plans (for their family member with a developmental disability) for a time when they are no longer able to care or provide direct support.
The Community Teams are committed to supporting and empowering individuals who use DD services so they may make informed decisions for themselves.
