Glossary of Terms
This glossary of terms will help you decipher the provider guides.
Adult Day Support services
Services provided separate from any home or facility in which an individual resides, focus on non-work activities, and include five components:
- Assessment - may be formal or informal, for the purpose of developing an Individual Service
Plan (ISP) - Personal care - includes personal hygiene, eating, communication, mobility, toileting, and dressing
- Skill reinforcement - includes implementing behavioral intervention plans, and help with the use
of communication and mobility devices - Training in self-determination - includes developing self-advocacy skills and acquiring skills that
enable an individual to become more independent - Recreation and leisure - includes supports identified in the ISP that are therapeutic, and help to
develop and maintain social relationships and family contacts

Agency Provider
A support person who has been hired and trained by an ODDD-certified agency.
Individual Options Waiver
A Medicaid waiver/funding source with an allotment over $5,000; amount determined through state assessment (ODDP). The waiver funds staff to provide services such as Homemaker/Personal Care, Day Habilitation, Respite, Leisure Recreation, etc. The waiver also may fund Equipment, Medical Supplies, Home Modifications, Emergency services, etc.
Homemaker/Personal Care
The coordinated provision of a variety of services, supports and supervision necessary for the health and welfare of an individual which enables the individual to live in the community. These are tasks directed at increasing the independence of the individual within his/her home or community.
Individual Provider
An ODDD-certified support person who operates as an independent agent, not hired by an agency.
Level One Waiver
A waiver/funding source with an allotment of $5,000 or under for staff to provide services such as Homemaker/Personal Care, Day Habilitation, Respite, Leisure Recreation, etc. The waiver also may fund Equipment, Medical Supplies, Home Modifications, Emergency services, etc.
Non-medical Transportation
A waiver service supplied by an agency or individual provider that allows transportation to employment and/or day-programming.
Respite
A service provided by an agency or individual provider that allows the primary caregiver of a person being supported a chance to take a break. Respite services can occur an evening or for a week – whatever the person’s support team decides is needed.
Supported Employment Services
Services that are intensive, ongoing supports that help people to perform work in
a regular employment setting, including self-employment. Supported Employment does not include sheltered
work or other vocational services furnished in specialized facilities.
There are two types of Supported Employment services:
- Enclave Work Option
Provided to individuals who work as a team at a single work-site (community business
or industry) with ongoing support provided by on-site staff
- Community Work Option
Provided to individuals who work in an integrated community work setting, along
side employees without disabilities, and performing same or similar tasks
Non-Medical Transportation includes transportation services necessary for an individual to participate
in Adult Day Support, Vocational Habilitation, and Supported Employment, as specified in an
Individual Service Plan (ISP).
Supported Living
Defined as assistance directed toward individuals with disabilities, which enables them to live as independently as possible in their own communities, with supports as they choose.
Transportation
A waiver service supplied by an agency or individual provider that allows the person being supported the opportunity to be driven to doctor appointments, as well as other community-based activities (leisure/recreation, grocery shopping, etc).
Vocational Habilitation Services
Teach and reinforce concepts related to work such as:
- Responsibility
- Attendance
- Task completion
- Problem solving
