About the Agency
The mission of Opportunites to Succeed is to provide quality services to individuals with mental retardation and develpmental disabiliities, while maintaining those traits which are unique to each individual. It is the believe of OTS, Inc. that each individual deserves excellent service and the opportunity to succeed in every aspect of life. Therefore, OTS will work collaboratively with all advocates, family members, employees, and community members to ensure that each individual served has the opportunity to achieve their greatest potential. OTS, Inc. will strive to achieve excellence of service, empower all individuals and employees to work together toward common goals, to maintain integrity and honesty, and to work with the community to increase awareness of the worth, as well as the need, of individuals with mental retardation and developmental disabilities. The agency currently serves 34 individuals in Butler County, three in Warren County and eight in Hamilton County. They have been providing services for the last seven years. OTS is a for-profit corporation.
Services provided
Opportunities to Succeed provides IO Waiver (Homemaker/Personal Care) services to 41 individuals living in Butler County. The agency also provides Level 1 Homemaker (Homemaker/Personal Care) services to 4 individuals, and provides transportation. New referrals are being accepted for IO Waiver and Transportation services.
Experience
Opportunities to Succeed has experience providing services to children and adults with the following mental retardation and/or developmental disability: autism, cerebral palsy, medically fragile, and/or physical impairment.
Direct Service Staff Information
Opportunities to Succeed requires all direct service staff to have a high school diploma, to have certification in CPR, first aid, medication administration and to have MUI training. The standard requirement for any employee is to successfully pass a background check, a nurse aid registry check, an an abuser registry check. Employees drive and are required to have a clean driving abstract. They are covered under the agency auto policy. For orientation into the agency, employees shadow current staff for at least one shift, they are given individual specific training, plus training in UI/MUI, home familiarity, individual rights, agency benefits and abuser registry. The average in-service training hours offered per year are 10. OTS has a full-time direct care staff of 65. The starting pay rate for staff is $8 an hour and employees are offered health, dental, and vision insurance and paid vacation.
Supervisory/Administrative Structure
The structure for administration is (from top down): president, director, residential supervisor, direct service manager, and direct service staff.
Certification
Opportunities to Succeed is certified as a supported living provider, an individual options waiver provider, a level 1 waiver provider, and as a transportation for supported living provider.
