About the Agency
The mission of Hopes Fulfilled is to transform statistics into stars by removing the barriers to achievement of self-determination, including freedom, authority, support and responsibility.

Services provided
Accepting new referrals for the following services: IO waiver, homemaker/personal care, Level 1 homemaker/personal care, foster care, day habiltiation/supported employment, planned respite care, emergency respite care, leisure/recreation, and transportation. The planned respite care capacity is from 2 to 8 individuals. The emergency respite care capacity is from 2 to 8 individuals.

Experience
Provides service in Butler and Hamilton County. Experience in providing services to children and adults with: autistic behavior, brain injury, hearing impairment, mental retardation, developmental disability, behavioral challenges, cerebral palsy, medically fragile, mental illness, physical impairment and visual impairment.

Direct Service Staff Information
The educationa/experience requirements for direct care staff are to have a high school diploma and MUI training. Certain positions require a two-year degree. Direct staff drive and have verified driver's licenses, plus annual verification for non-medical transportation status plus physician statements, substance testing, training on safe vehicle operation including wheelchair life and operation of adaptive equipment. Standard requirements for all direct care staff are to be 18 years of age and older, have a BCII background check, nurse aide registry check and an abuser registry check. Average in-service training hours per year is 20. Number of direct service staff is three. Staff length of service is five years or more. The pay range of staff is $8-9 as a starting salary, depending on experience. Benefits are offered contractually.

Supervisory/Administrative Structure
The agency is comprised of a Program Director and Program Coordinators who provide guidance and support to direct service staff.
